Digital gym waivers: get terms accepted without paper forms

Replace paper waivers with a one-time link: write your terms once, send them on WhatsApp, and keep a dated record of every member who read and accepted them.

Most gyms have a liability waiver or a set of house rules somewhere. In practice it is a stack of printed sheets in a drawer, half of them unsigned, and the one you need after an incident is never the one you can find. Updating the rules means reprinting everything, and a member who joined last year signed a version nobody remembers.

GymLeb's Member Forms replace that drawer. You write your waivers, terms and agreements once, send a member a one-time link, and GymLeb keeps a dated copy of exactly what they accepted. This guide covers how to write a form, send it and find the record later.

What a member form is

A member form is a document the member reads and accepts: a liability waiver, gym rules, a PT agreement, a freeze policy, a photo consent, a minors' consent for parents. The member opens a link, reads the text, ticks a box saying they accept, and presses Accept. They can download a PDF copy for themselves.

Forms are for reading and accepting. They do not collect information through input fields, so they are not a sign-up form or a health questionnaire. Member details and health information are entered on the member's profile, by your staff or by the member in the member app.

Write your forms once

Open Member Forms in the sidebar. Each form is built from simple blocks: headings, paragraphs and lists. A live preview sits next to the editor, so you see exactly what the member will see. You can also set the text next to the accept box; by default it says "I have read and accept the terms above".

Forms can use placeholders that are filled in when you send them: the member's name and phone, your gym's name and phone, and the date. A waiver that starts with the member's own name reads very differently from a generic one.

If you do not have your terms written yet, Build with ChatGPT copies a ready prompt you can paste into ChatGPT, then checks the text you paste back so it fits the editor. Nothing is sent to an AI service from GymLeb; you stay in control of what goes into your waiver. Either way, have your final text reviewed by someone who knows the law where you operate.

Tick Can be sent to members on the forms that are ready to use.

Send a form to a member

On the member's page, open the Forms tab and press Send form. Choose the form, then either:

  • copy the link and send it however you like, or
  • tick Also send the link by WhatsApp to message it straight to the member.

The link is shown once, so copy it before closing the dialog. Sending by WhatsApp uses one message from your allowance and requires the Member form link message to be switched on in your WhatsApp settings. Our guide to WhatsApp messages explains the allowance.

The member opens the link on their phone without signing in. The page shows your gym's logo, name, phone and address, the form, and the accept box.

Track who accepted what

Every sent form has a status on the member's Forms tab: Sent, Opened, Accepted, Expired or Revoked. View shows the accepted copy with the date and time of acceptance, plus the IP address and browser it was accepted from, and you can print it or save it as PDF.

Importantly, the text is saved as it was when you sent it. If you update your rules next year, members who accepted the old version still show the old version, which is exactly what you want when someone asks "what did I agree to?".

If a member lost the link, Send again creates a new one and the old one stops working. Revoke cancels a link you sent by mistake.

  • One-time use. The link can be opened as many times as needed until it is accepted; after that it is used up.
  • It expires after 7 days. An expired, used or revoked link shows a clear message instead of the form.
  • It is private. The page is never cached or indexed by search engines, and repeated wrong attempts are blocked.

Forms worth setting up

  • Liability waiver: the basics of training at your own risk.
  • House rules: towels, equipment, opening hours, ladies' hours, guests.
  • Freeze and refund policy: when a membership can be frozen and what happens on cancellation. Clear terms prevent the most common desk arguments.
  • Personal training agreement: late cancellations, no-shows and package expiry. See how GymLeb handles those in our personal training guide.
  • Photo and social media consent if you post member photos.
  • Parental consent for members under 18.

A simple routine

  1. Write two or three forms this week, starting with the waiver and house rules.
  2. Send them to every new member on the day they join, while they are still at the desk.
  3. Send the waiver to existing members over the next month, a group at a time.
  4. When a rule changes, edit the form and send the new version to everyone affected.

Member forms are part of GymLeb; sending them by WhatsApp uses your message allowance. See the pricing page for plans, or contact us if you want help turning your paper forms into templates. If you are moving your whole gym over, our switching guide is the next read.
